Take a Break, Before you Break: Family Caregiver Survey (extended)

The Nebraska Planning Council on Developmental Disabilities has awarded a grant to form a Respite Taskforce to address the problems family caregivers have using respite care services.

A Respite Taskforce has been formed to collect information from families caring for loved ones of all ages, special needs or chronic health conditions.  Your experience is vital in assisting us in making recommendations  to those who can potentially do something about the challenges and barriers you may have faced.  Respite is defined in this case as providing a short-term break to those caring for family members with special needs.  Respite can be provided through a variety of financial sources through the Nebraska Department of Health and Human Services (DHHS) and the Veteran’s Administration (VA).

We greatly appreciate your answers to the following questions by April 6, 2018.  If you feel like sharing your personal story, please take time to offer this as well.
Your answers will help us to better serve you.
